Upon accessing the page, you will be presented with a list of forms. Simply choose the form you wish to fill out. This will pull up a list of approved submissions from all workers. If you click on the "My Submissions" screen, you will be able to view all your submissions for this particular form.

Located at the top right corner of the page, you will find a "New Submission" tab. By clicking on this tab, you will be able to upload a completed form directly from your computer. Remember to save your uploads to ensure that the submission is successfully recorded. Just below the "New Submission" tab, you will find an option to download a blank form. This allows workers to access and complete the form offline, before uploading it as a submission.
