Formstack is a web-based form tool that has some fantastic customizations without breaking the bank.

After creating your Formstack account you can head to Forms and create a new form. The drag and drop interface is simple to use and the advanced question options are some of the best we've seen.

Once complete head to the Settings>Email & Actions menu:

You can edit the existing Notification Email or delete it and create a new one.
Setup the TO: for webforms@workhub.com and enter the FormID=## in the subject
line:

You can see that in this case I dragged a few variables over to submit additional data into Workhub.

Save Notification Email once complete:

Lastly, to make the form available to your workers directly from Workhub, head to the General menu and grab the Form URL to paste onto the form details page in Workhub:
